Job summary

Optimist Youth Homes and Family Services is seeking a Case Manager to support children and adults in the APSS program by connecting them with resources, helping navigate the adoption process, and assisting with meeting needs while in the program.

Responsibilities include conducting intake, coordinating with agencies and community partners, developing needs assessments, documenting SMART goals, and providing family-centered, strength-based advocacy and parenting education.

Qualifications

• Intake for referred children or adults.
• Accesses community resources per family needs.
• Ability to coordinate with social service agencies and schools.
