Job summary

Optimist Youth Homes and Family Services is seeking a Case Manager to support children and adults in the APSS program by connecting them with resources, helping navigate the adoption process, and assisting with meeting needs while in the program.

Responsibilities include conducting intake, coordinating with agencies and community partners, developing needs assessments, documenting SMART goals, and providing family-centered, strength-based advocacy and parenting education.

Position Summary

The Case Manager provides support for children and adults participating in the APSS program by connecting them with resources, helping them navigate the adoption process, and assisting with meeting or finding resources to meet their needs while in the program.

Essential Duties And Responsibilities Include

• Conduct intake process for newly referred children or adults.
• Accesses community-based resources appropriate to the family’s needs.
• Interfaces with Department of Children and Family Services, Department of Mental Health, court systems, schools, health care providers and other community liaisons.
• Assists family with developing a needs/strength assessment.
• Provides support and advocacy which are family-centered, community and strength-based.
• Provide Adoption-Focused parenting education for adults pursuing adopting a child through the APSS program.
• Documents client’s strengths and needs; facilitates the development of measurable goals and objectives related to client strengths and needs.
• Creates a plan of activities to be accomplished with the client and documents S.M.A.R.T. goals.
• Facilitates a monthly parenting support group and collaborates with team members regarding its implementation and promotes participation from clients.
• Provides parenting education.
• Follows states laws and agency guidelines regarding child abuse reporting.
• Maintains weekly service provision requirements.
• Active participation in administrative and clinical supervision and meetings.
• Is receptive in receiving feedback from children and families, community and service providers.
• Ensures accuracy in work and that all deadline expectations are met.
• Maintains consistent and regular attendance.
• Follows up with supervisor in a timely manner regarding significant issues.
• Demonstrates ownership of work responsibilities by incorporating feedback, taking initiative, and interacting in a professional manner within assigned role and adhering to agency guidelines.
• Maintains confidentiality as required by PLYS, licensing, HIPAA, and other regulatory agencies.
• Follows and adheres to all field safety guidelines.
• Other duties as assigned.
