Rehabilitation Center of Independence West Campus seeks an experienced MDS Coordinator to join a dedicated team providing high-quality resident care. You will lead the care-planning process, ensure accurate assessments, and support discharge planning to optimize resident outcomes.

The ideal candidate holds an Iowa RN license, has completed an accredited nursing program, and possesses CPR certification, with prior MDS coordination and SNF RN experience preferred.

Qualifications

• Iowa RN license in good standing.
• Graduate of an accredited nursing program; CPR certification.
• Prior MDS Coordinator experience; SNF RN experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments to determine needs.
• Assist in the discharge process and coordinate with care teams.
