Job summary

Rehabilitation Center of Independence West Campus is seeking an experienced MDS Coordinator to join our caring skilled nursing team. You will develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals and assist with the discharge process.

To be eligible, applicants must hold an unencumbered Iowa RN license, be a graduate of an accredited nursing program, and CPR certification.

Qualifications

• Unencumbered State of Iowa RN License.
• Graduate of an accredited nursing program.
• CPR Certification.
• Prior experience as MDS Coordinator; RN in SNF for 1 year and CNA for 1 year preferable, but not required.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals, and assist in discharge planning.
• Provide leadership and coordination to ensure high-quality care for residents and regulatory compliance.
