Qualifications

Responsibilities

• Perform pre-admission assessments of residents and coordinate discharge planning.
• Attend facility meetings and participate in in-service training.
• Coordinate admissions and discharge planning with families, physicians, and staff.
• Maintain referral tracker and assist with care plan meetings.
• Promote positive public relations with residents, families, and referral sources.
• Support nurse-on-call rotation and ensure resident-centered care.
