Job summary

Linden Woods Village in Gladstone, MO is seeking a dedicated Admissions Coordinator to assess, plan, and advocate for residents to reach optimal physical, mental, and psychosocial well‑being. You will handle pre‑admission assessments and discharge planning for residents with effective communication to families and care teams.

The role supports a nurse‑led team in a continuing care retirement community, with a focus on resident‑centered care and collaboration across physicians and allied health

Qualifications

• Graduate from an accredited school and hold an active RN or LPN license.
• If an LPN, must enroll in an RN program within 6 months and complete it.
• Preferred 2 years of geriatric experience.
• Must have a valid driver’s license.

Responsibilities

• Follows the community’s philosophy and goals to enhance residents’ quality of life.
• Perform pre‑assessment of residents as needed.
• Attend facility meetings and in‑service trainings.
• Participate in nurse on‑call rotation.
• Coordinate admissions and discharge planning.
• Facilitate care plan meetings and advocate for resident‑centered care.
• Communicate with families, physicians, and care team.
