Linden Woods Village in Gladstone, MO is seeking a dedicated Admissions Coordinator to assess, plan, and advocate for residents to reach optimal physical, mental, and psychosocial well‑being. You will handle pre‑admission assessments and discharge planning for residents with effective communication to families and care teams.

The role supports a nurse‑led team in a continuing care retirement community, with a focus on resident‑centered care and collaboration across physicians and allied health

Qualifications

• Graduate from an accredited school and hold an active RN or LPN license.
• If an LPN, must enroll in an RN program within 6 months and complete it.
• Preferred 2 years of geriatric experience.
• Must have a valid driver’s license.

Responsabilités

Role:

We are seeking a dedicatedAdmissions Coordinatorto join our team!The Admissions Coordinator makes assessment for, plans and advocates for the residents of the Communityso they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.

Responsibilities:

• Follows and upholds the Community’sphilosophy and goal to maintain, improve and/or enhance each resident’s quality of care and quality of life
• Perform pre-assessment of residents as necessary.
• Attends and participates in facility meetings and in-service training programs as assigned.
• Participates in the nurse on-call rotation
• Promotes positive public relations with residents, family members, guests, and outside referral sources.
• Collects and assesses data relevant to resident’s psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.
• Completes assigned portions of the MDS.
• Maintains referral tracker.
• Coordinates admissions and discharge planning
• Reports monthly discharges to the Ombudsman.
• Coordinates and directs the care plan meetings.
• Facilitates advanced directive decision-making process
• Coordinates grievance complaint process
• Coordinate and assist with resourcing and scheduling of allied health services i.e. Dentists, Podiatrists, Medical specialists etc.

Qualifications: Qualifications:

• Must be a graduate from an accredited school, be currently licensed RN or LPN in state of which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements.If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration. Proof of enrollment and documentation confirming the program’s expected completion timeframe must be provided upon request.
• Prefer 2 year experience working in a geriatric setting.
• Must possess a valid drivers license
