Qualifications

Responsibilities

• Support the mental and physical health care of patients on an assigned caseload.
• Screen and assess patients for mental health and substance use disorders; facilitate engagement and follow-up care.
• Provide patient education about mental health and substance use disorders and treatment options.
• Track treatment response and monitor patients in person or by telephone for clinical changes and complications.
• Support psychotropic medication management by consulting with the behavioral health consultant and communicating with the primary care provider.
• Provide brief behavioral interventions using evidence-based techniques (behavioral activation, problem-solving treatment, motivational interviewing).
• Provide or facilitate referrals to evidence-based psychosocial treatments.
• Participate in weekly caseload consultations with psychiatric consultants and communicate treatment recommendations to medical providers.
• Track patient follow-up and clinical outcomes using a registry; document encounters.
• Document patient progress and treatment recommendations in the EHR and required systems.
• Facilitate treatment plan changes and referrals for more intensive care.
• Facilitate referrals for outside services such as housing, vocational rehab, specialty mental health care, and substance abuse treatment.
• Develop relapse prevention self-management plans for patients completing treatment.
• Utilize Excel and Word and complete daily assessments and productivity trackers.
• Manage daily work using calendars and the EMR system.
• Conduct primarily telephonic work as well as in-person visits.

Requirements

• The Social Worker Care Manager will have a minimum of MSW


• Preferred Licensure or provisionally licensed


• Behavioral Health experience and knowledge


• Must be licensed driver with an insured automobile in good working order


• Ability to communicate effectively, both verbally and in writing.


• Time Management skills


• Prior behavioral health experience is helpful but not required.
