Job summary

BAYADA Home Health Care in Hillside, NJ seeks an Admissions Registered Nurse (RN) for full-time home health admissions and OASIS Start of Care visits across Monmouth, Middlesex, Union, and Somerset Counties. You will assess, plan care, and coordinate with physicians, clients, and families to ensure a smooth transition to BAYADA services.

Ideal candidates have NJ RN licensure, at least one year of clinical nursing experience, and prior home health/OASIS experience.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Responsibilities

• Perform comprehensive initial nursing assessments and establish individualized plans of care.
• Coordinate with physicians, clients, caregivers, and inter-disciplinary team for seamless transitions.
• Educate clients and caregivers about diagnoses, medications, and home health plan of care.
• Document assessments and care coordination activities in the EMR system.
• Ensure timely completion of OASIS SOC admissions and related documentation.
