Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for a full-time role covering Monmouth, Middlesex, Union and Somerset Counties in NJ. You will lead OASIS SOC admissions and initial assessments, creating individualized care plans for adult and geriatric clients.

Qualifications

• Current New Jersey RN license.
• Minimum of one year of verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.

Responsibilities

• Conduct comprehensive OASIS Start of Care admissions visits for home health clients.
• Perform thorough initial nursing assessments and medication reconciliation.
• Develop individualized plans of care in collaboration with physicians and the inter-disciplinary team.
• Educate clients and caregivers about diagnoses, medications, safety, and the plan of care.
• Document assessments and care coordination in BAYADA's EMR and ensure accurate admission paperwork.
