BAYADA Home Health Care is seeking a Full Time Admissions Registered Nurse to perform Start of Care visits for adult and geriatric patients across northeast Massachusetts. Medicare-certified home health and OASIS documentation experience are required.

You will work with a multidisciplinary team to deliver skilled nursing in patients’ homes, emphasizing quality, compassion, and reliability. This role offers paid training and 24/7 clinical support.

Qualifications

• Current nursing license in good standing in MA.
• Graduation from an accredited nursing program as indicated by transcript/diploma.
• At least 1 year of adult/geriatric clinical experience in acute care, ED, home health, hospice, ICU, med-surg, or rehab.

Responsabilités

• Perform Start of Care (SOC) OASIS visits for adult/geriatric patients in northeast Massachusetts.
• Independently triage and manage urgent clinical situations in home settings.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.
• Maintain high quality, compassionate home health nursing while coordinating with the care team.
