Job summary

Select Home Health is seeking a skilled and compassionate Registered Nurse to provide in-home nursing care throughout the Indianapolis, Indiana area. This is a float position that requires flexibility to provide coverage across multiple territories.

The RN will deliver high-quality, personalized care while helping patients maintain their independence and overall well-being. This role focuses on patient assessments, implementing physician orders, education for patients and caregivers, and

Qualifications

• Graduate of an accredited school of professional nursing.
• Indiana RN license (required).
• Minimum of 1 year nursing experience in hospital, facility, or home health.
• Current TB test.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
