Moments Hospice in Wisconsin is seeking an Admissions RN to join our team on a full-time, on-the-road basis. The role includes a company car with fuel and insurance coverage and a schedule Mon–Fri, 12:00 pm–8:00 pm.

Responsibilities center on admissions processes, patient evaluation for hospice suitability, and building strong referral networks. The ideal candidate holds a valid RN license, with 1–2 years of nursing experience and a compassionate, patient-centered approach.

Qualifikationen

• RN license required.
• Driver's license with vehicle provided.
• 1–2 years nursing experience preferred.
• Licensed RN in state of performance.
• Experience in hospital, physician practice, LTC or ALF is advantageous.

• Maintain clinical and professional competence.
• Develop internal relationships with admissions and patient care teams.

Nurses looking for autonomy and variety should consider practicing in Hospice Care. As an Admissions RN, you will join a team that is dedicated to making our patients' final days, weeks, and months as comfortable as possible.

Hours for this position will be Monday-Friday 12:00 pm-8:00 pm

Responsibilities:

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
• Obtain agreement for admission of referred patients and families who meet admission criteria.
• Analyze data collected to facilitate/obtain admission orders
• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Moments Hospice
• Maintain clinical and professional competence.
• Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient's family has concerning hospice and palliative care

Requirements:

• Valid Registered Nurse (RN) license
• Driver's license, insurance and vehicle
• 1-2 years of nursing experience (preferred)
• Utmost compassion and integrity
• Currently licensed as a R.N. in the state where duties are performed.
• Previous hospital, physician practice, LTC or ALF experience
• Strong Presentation Skills
• Previous sales or service experience preferred
