Job summary

St. Joseph Hospice in Mobile, AL is hiring a Hospice Admissions Registered Nurse (RN) for daytime to evening shifts (11am-7pm).

You will conduct initial and ongoing patient assessments, help develop the Plan of Care, and serve as primary liaison between physicians, patients, families and the hospice team. This role emphasizes education on the Plan of Care, medication administration, and comfort-focused care.

Qualifications

• Must maintain Alabama RN license and CPR certification.
• Hospice experience preferred.
• Ability to make patient visits at residences or facilities.
• On-call rotation required; weekly call may be needed.

Responsibilities

• Perform the initial and ongoing assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medications, dressings, oxygen.

Job Duties

• Performs the initial and on-going assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
• Act as primary liaison between physician, patient/family and hospice team
• Maintain an accurate and updated medical record
• Makes routine supervisory visits
• Participates in agency on-call schedule
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards

Qualifications

• Registered Nurse (RN) license required in State of Alabama
• Current CPR certification required
• Make patient visits at their place of residence or facility
• Call will be required once a week as needed
• Knowledge of EMR is required
• Previous Hospice experience needed
• Compassionate RN with great time management skills
