St. Joseph Hospice is actively hiring a Hospice Admissions Registered Nurse (RN) to support patients in Mobile, AL.

The role involves conducting initial and ongoing patient assessments and coordinating the Plan of Care across the hospice team, physicians, and families. Responsibilities include educating patients and families, maintaining meticulous medical records, performing on-call duties weekly, and ensuring compliance with policies and regulations while delivering compassionate,

Qualifications

• Must hold an active Alabama RN license.
• Current CPR certification required.
• Experience in Hospice or end-of-life care preferred.
• Ability to visit patients at homes or facilities.
• Proficient with EMR systems.

Responsibilities

• Performs the initial and ongoing assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medication administration.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Participate in agency on-call schedule.
