The Arc Southern Chesapeake is seeking a Supports and Services Specialist to lead admissions, referrals, and transition processes, ensuring placement reflects authorized funding, eligibility, and the needs of individuals served. This role also supports development of Person-Centered Plans (PCP) using CQL’s POM to identify meaningful outcomes and monitor progress.

This hybrid role covers the Southern Chesapeake region including Calvert, Charles, St.

Formación

• Bachelor's degree in Human Services is required or related field.
• Minimum of three years relevant experience preferred.
• Knowledge of Maryland DDA service delivery and legal/regulatory requirements is desirable.

Responsabilidades

• Lead admissions process from referral through placement ensuring funding and eligibility align with needs of individuals.
• Coordinate and track PCP development and transitions using CQL’s Personal Outcome Measures (POM).
• Facilitate stakeholder communication and maintain admissions documentation and reports.
• Collaborate with teams to improve admission timelines and overall client experience.
