Responsibilities

• Support census growth and deliver an exceptional experience for residents, families, and referral partners.
• Develop and maintain referral relationships with hospitals, rehab centers, physicians, and other partners.
• Travel regularly to referral sources to strengthen relationships and opportunities.
• Coordinate the admissions process from inquiry to move-in.
• Conduct warm, professional resident and family tours with follow-up.
• Review referrals for payer source, insurance verification, and admission opportunities.
• Partner with clinical, business office, and leadership teams for smooth transitions.
• Maintain consistent communication with families and hospital teams.
• Track referrals, conversions, denials, and census-related metrics.
• Support census improvement strategies and outreach plans.
• Document referral activity and follow-up actions accurately and timely.
