Qualifikationen

• Must possess a current unencumbered state RN or LPN license.
• CPR certification required.
• One year of professional nursing experience in long-term care.

• Convert referrals to admissions and track status daily.
• Communicate with external team members for referral updates.
• Review referrals for medical approval and document findings.
• Consult with Nursing leadership to assess facility capabilities.
• Transcribe physician orders and verify accuracy of data.
• Evaluate Medicare Part A and Medicaid criteria before admission.
• Identify and document diagnoses codes for admissions/readmissions.
• Maintain ICD-10 coding knowledge and pay status awareness.
• Support SNAs and other staff as needed in admissions process.
• Collaborate with interdisciplinary team to resolve issues.
