Lutheran Homes of Michigan, Inc. in Saginaw is seeking a Campus Admissions & Clinical Documentation Coordinator (RN/LPN) for a full-time, on-site role.

You will guide families through admissions, coordinate bed availability, and ensure smooth transitions into the community while applying your clinical knowledge to support documentation and insurance processes. As the first point of contact, you’ll conduct tours, collaborate with nursing leadership, therapy teams, and MDS to ensure accurate

Qualifications

• Current Michigan Licensed Practical Nurse (LPN) license OR Registered Nurse (RN) license, required.
• Minimum of two years of skilled nursing, rehabilitation, or long-term care experience preferred.
• Strong communication and customer service skills.
• Experience with clinical documentation, insurance processes, admissions, or care coordination is highly valued.
• Ability to work collaboratively with residents, families, nursing teams, and healthcare professionals.
• Strong organizational skills and attention to detail.

Responsibilities

• Guide families through the admissions process and ensure they feel supported during transitions.
• Coordinate admissions, discharges, and bed availability with nursing leadership and other teams.
• Prepare for admissions by ensuring rooms and campus teams are ready to welcome residents.
• Conduct tours and act as an ambassador for the community.
• Support documentation processes, insurance authorizations, Medicare certifications, and MDS communications.
• Collaborate with interdisciplinary teams to ensure accurate and timely reporting throughout a resident's stay.
