Job summary

Qualifications

Responsibilities

• Coordinate and manage the admission process for patients entering the hospital.
• Conduct initial assessments and determine eligibility and level of care.
• Collaborate with physicians, nursing staff, social workers, and other professionals to facilitate admissions.
• Ensure admission documentation is complete and compliant with policies.
• Communicate with patients, families, and referrals to provide information and support.
• Maintain up-to-date knowledge of hospital services and policies related to admissions.
• Assist in discharge planning and coordinate with case management.
