RN licensure in NY preferred; LPNs with relevant experience are encouraged to apply. On-call rotation and collaboration with the DON/ADON team are required to ensure timely admissions and accurate documentation.

Qualifikationen

• Active NY RN or LPN license in good standing
• RN preferred; LPN with relevant experience encouraged to apply
• Experience in skilled nursing, long-term care, rehab, or hospital admissions preferred
• EMR experience with PointClickCare a plus
• Strong clinical assessment, organizational, communication, and interpersonal skills

Aufgaben

• Coordinate admissions from referral through resident arrival and follow-up
• Review referrals, medical records, medications, and orders for accuracy
• Conduct admission nursing assessments and identify care needs
• Communicate with physicians, hospitals, referral sources, residents and families to facilitate admissions
• Collaborate with DON/ADON and interdisciplinary team to support continuity of care
• Complete admission paperwork and EMR entries in PointClickCare
• Ensure records are accurate and compliant with policies and regulations
• Provide guidance to residents and families during transition
• Participate in on-call rotation as required
• Perform additional duties as assigned by DON/ADON
