Aufgaben

• Complete admission assessments for new residents and hospital returns, including initial plan of care.
• Support and guide the Nursing Department as directed by the Director of Nursing.
• Review PRI information and coordinate admission with the interdisciplinary team.
• Review resident records and perform audits to ensure standards are followed.
• Update care plans with changes in condition.
• Assist with discharge planning as needed.
• Complete all required documentation and charting timely.
• Complete MDS assessments, care plans, and other required paperwork.
• Oversee and direct resident care when needed.
• Maintain HIPAA compliance and confidentiality.
