Qualifications

• RN Admissions Nurse with a valid RN license in the state of New York.
• Experience in geriatric care.

Responsibilities

• Conduct admissions assessments and develop individualized care plans in collaboration with the care team
• Coordinate with healthcare professionals and community resources to ensure a comprehensive care approach
• Provide education and support to residents and their families, promoting independence and empowerment
• Collaborate with the interdisciplinary team to identify and address potential barriers to care
• Maintain accurate and up-to-date records and documentation
• Participate in quality improvement initiatives to enhance the care experience
• RN Admissions Nurse will also be responsible for other duties as assigned by the DON or deisgnee
