Sangre de Cristo Community Care is seeking an experienced Registered Nurse to grow into the role of Assistant Clinical Manager, partnering with the Clinical Manager to support Hospice and Home Health teams. This position is ideal for an RN Case Manager or clinical leader looking to expand leadership skills in a collaborative, mission-driven environment.

Qualifications

• Current Colorado RN license.
• Minimum of three years Hospice and/or Home Health experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organizational, and leadership abilities.
• Previous supervisory, charge nurse, preceptor, or leadership experience preferred.
• Valid drivers license and reliable transportation.

Responsibilities

• Partner with the Clinical Manager in the daily oversight of Hospice and Home Health clinical operations.
• Provide clinical guidance, mentoring, and support to nurses and interdisciplinary team members.
• Assist with staff orientation, onboarding, and ongoing competency development.
• Help coordinate clinician schedules and patient assignments to ensure quality care and efficient operations.
• Participate in interdisciplinary team meetings and case conferences.
• Monitor documentation quality, regulatory compliance, and clinical best practices.
• Conduct chart audits and assist with quality improvement initiatives.
• Serve as a resource for complex clinical situations and patient care concerns.
• Maintain a reduced patient caseload while supporting leadership responsibilities.
• Assume delegated leadership responsibilities in the absence of the Clinical Manager.

Requirements

• Current Colorado Registered Nurse (RN) license
• Minimum of three years of Hospice and/or Home Health experience
• Strong clinical assessment and critical thinking skills
• Excellent communication, organizational, and leadership abilities
• Previous supervisory, charge nurse, preceptor, or leadership experience preferred
• Knowledge of Medicare Conditions of Participation and regulatory requirements preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and proof of automobile insurance
