Job summary

Sangre de Cristo Community Care is seeking a Float RN to provide clinical support across our program. The role includes admissions, patient visits, and coverage for RN Case Managers during PTO and staffing needs, ensuring continuity of care across the team.

• Hospice, Home Health, Oncology, Palliative Care, or ICU experience preferred.

Responsibilities

• Completing hospice admissions and start-of-care visits.
• Providing routine and PRN patient visits.
• Covering RN Case Manager caseloads during PTO/absences/education.
• Assisting with symptom management and changes in patient condition.
• Collaborating with physicians and the interdisciplinary team to develop and update care plans.
• Educating patients and families on disease progression, medications, and end-of-life care.
• Supporting timely response to changing patient needs.
• Documenting all patient care in the EMR.
• Maintaining continuity of care across the team.

In this dynamic role, you'll help ensure continuity of care by assisting with admissions, patient visits, and providing coverage for RN Case Managers during PTO, education, and other staffing needs.

As a Float RN, you'll serve as a versatile clinical resource by:

• Assisting with symptom management and changes in patient condition
• Collaborating with physicians and the interdisciplinary team to develop and update individualized care plans
• Educating patients and families on disease progression, medications, and end-of-life care
• Supporting timely response to changing patient needs
• Accurately documenting all patient care in the electronic medical record
• Helping maintain excellent continuity of care across the team
