Job summary

Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN seeks an Admissions Liaison to grow census by cultivating hospital and community referrals and guiding residents and families through transitions.

The role requires building partnerships, conducting tours, coordinating admissions, and handling inquiries with a focus on collaboration with the Admissions Director and care team.

Qualifications

• Experience in marketing and admissions for a skilled nursing facility.
• Long-term Care/skilled Nursing experience preferred.
• Strong communication, presentation, and relationship-building skills.
• LPN/RN license preferred.
• Ability to work flexibly, including some evenings or weekends.

Responsibilities

• Build and maintain relationships with hospitals, discharge planners, physicians, and other referral sources.
• Conduct facility tours and presentations for prospective residents and families.
• Coordinate admissions paperwork and processes with the Admissions Director and interdisciplinary team.
• Handle inquiry calls from hospitals, families, and community organizations and schedule follow-up appointments and tours.
• Represent the facility at community events and outreach activities to promote services and build trust.
• Perform other marketing and admissions liaison duties as assigned by management.
