Job summary

Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN is seeking an experienced Registered Nurse to coordinate and deliver resident care, leading an interdisciplinary team in a skilled nursing setting.

The RN will assess, plan, implement, and evaluate individualized care, delegating tasks appropriately and collaborating with physicians, therapists, and social services to ensure quality, person-centered services and successful discharge planning.

• Strong clinical assessment, leadership, and communication skills.

Responsibilities

• Complete comprehensive and focused assessments and develop, implement, and update individualized care plans.
• Provide and oversee direct nursing care, including treatments, medication administration, and clinical interventions.
• Lead and support LPNs, CNAs, and CMAs, ensuring safe delegation and follow-through.
• Collaborate with physicians, therapists, social services, and other disciplines to coordinate resident care and discharge planning.
• Communicate with residents and families regarding care plans, changes in condition, and education needs.
• Ensure accurate, timely documentation and participate in quality improvement and regulatory compliance activities.
• Promote a culture of safety, teamwork, and resident-centered care.
