Job summary

FutureCare Admissions In-Take Nurse in Maryland reviews pre-admission information, coordinates referrals with NaviHealth, Allscripts and other systems, and supports admission processes with hospital staff and families. The role requires collaboration with leaders to ensure accurate screening and timely responses.

Requires current Maryland RN or LPN license and at least one year in admissions/long-term or sub-acute care; strong communication skills are essential.

Responsibilities

• Conducts timely review of pre-admission screening of potential patients, including admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information.
• Manage electronic referral systems with Admission Coordinator to ensure timely responses to referrals and communicate with referral sources.
• Communicate effectively with DON, Regional Care Managers, Admissions, Executive Director, Respiratory Director, and hospital staff, patients and families.
• Provide oversight and training for the Admission Coordinator.
• May contact payer sources to determine coverage and funding for services prior to admission.
• Attend regional meetings and in-service programs as directed; support continuous process improvements.
• Handle unusual occurrences calmly and perform miscellaneous tasks as needed.

The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc.

Responsibilities

• Conducts timely review of pre-admission screening of potential patients, to include but not limited to, admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information.


• Along with Admission Coordinator, manage electronic referral systems (NaviHealth, Aiden, Allscripts, ETC) to ensure timely responses to referrals, and maintain effective communication with referral sources. Monitor and analyze data from reports.


• Communicate effectively with DON, Regional Care Managers, Admissions, Executive Director, Respiratory Director, and other team members, as well as hospital staff, patients and families.


• Provide oversite and training for the Admission Coordinator


• May contact payer source to determine or verify coverage and adequate funding for required patient services such as high cost drugs, DME or other necessary services and length of stay prior to admission


• May attend regional meetings, scheduled in-service programs, and other necessary meetings as directed by Executive Director and VP Business Development.


• Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride


• Performs any miscellaneous work assignments as may be required

Qualifications

• Must have a current RN or LPN license in the state of Maryland


• Minimum one year experience in Admissions, long-term care and/or sub acute care


• Knowledge of ventilator and dialysis care preferred


• Strong oral and written communication skills, required


• Ability to sit, stand, and walk most of the time; may also require driving (Minimal travel as assigned by Director of Nursing
