Complete Care in Chicago seeks an Admissions Director to lead the admissions process, drive census growth, and manage referrals with healthcare providers and community partners. You will oversee onboarding, documentation, and pre-authorizations, while ensuring excellent resident transitions.

The role requires strong relationship-building, team collaboration, and a commitment to patient safety and HIPAA compliance.

Qualificações

• Experience in high-volume healthcare admissions.
• Strong knowledge of admissions documentation and patient safety regulations.
• Excellent interpersonal, communication, and relationship-building skills.
• Ability to work collaboratively within a team and handle cross-coverage duties.
• Commitment to confidentiality and ethical conduct.

Responsabilidades

• Promptly review and respond to referrals from hospital case managers, social workers, physicians, and families.
• Evaluate applications, medical records, and health conditions against admission criteria to determine acceptance or referral.
• Complete admission and financial responsibility forms, contracts, and packets accurately and timely.
• Verify health insurance, benefits, and pre-authorizations (Medicare/Medicaid/commercial) prior to placement.
• Serve as primary contact during transition; conduct tours and address concerns.
• Assist with pre-admission assessments for assisted living and independent living residents when requested.
• Compile, update, and submit weekly admissions and referral reports to track metrics and inform leadership.
• Ensure compliance with HIPAA and other regulatory guidelines.
