Job summary

Ambassador Healthcare at College Park in Fort Myers, FL is seeking an Admissions Coordinator to serve as the primary liaison between hospitals, residents, families, and referral sources. The role drives a seamless admissions process and supports occupancy growth with exceptional customer service.

The successful candidate will manage referrals through admission, verify insurance requirements, and stay current on Medicare/Medicaid policies.

Qualifications

• Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF) required.
• Strong knowledge of Medicare/Medicaid, PDPM, HMO guidelines, and RUGS II & III.
• Experience with Electronic Medical Record (EMR) systems.
• Knowledge of CMS regulations and Department of Health (DOH) requirements.
• Excellent communication, organizational, and relationship-building skills.
• Ability to multitask in a fast-paced healthcare environment.

Responsibilities

• Coordinate admissions from referral through admission.
• Collaborate with hospitals, physicians, case managers, and referral sources.
• Develop and maintain relationships with referral partners and community organizations.
• Support marketing and outreach by attending networking events and community functions.
• Educate prospective residents and families regarding services, policies, visitor guidelines, and admission procedures.
• Verify insurance coverage, financial information, and authorization requirements prior to admission.
• Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer-specific guidelines.
• Monitor admissions activity and prepare monthly reports on admissions trends and census growth.
• Identify operational barriers affecting admissions and work with leadership to implement solutions.
• Communicate admission details effectively with interdisciplinary team members to ensure continuity of care.
