Qualifications

• Minimum 2 years admissions experience in SNF.
• Strong understanding of Medicare/Medicaid/PDPM guidelines.
• Experience with EMR systems.
• Knowledge of CMS regulations and DOH requirements.
• Excellent communication and organizational skills.
• Ability to multitask in a fast-paced healthcare setting.

Responsibilities

• Coordinate admissions from referral to admission.
• Collaborate with hospitals, physicians, case managers, and referral sources.
• Develop relationships with referral partners and community organizations.
• Support marketing and outreach at events.
• Educate residents and families on services, policies, and admission procedures.
• Verify insurance and payment information prior to admission.
