Richmond Post Acute in Richmond, VA seeks an experienced Admissions Director to lead the admissions process from inquiry to discharge, coordinating with nursing staff, patients, and families to ensure a smooth transition and high-quality care.

The successful candidate will develop relationships with referral sources, analyze admissions data for process improvements, and collaborate with the Director of Nursing to align admissions with clinical operations while ensuring regulatory compliance.

Qualifications

• Bachelor's degree required.
• Minimum 2 years in admissions in a skilled nursing or LTC setting.

Responsibilities

• Manage the admissions process, including coordinating with nursing staff, patients, and families to ensure a smooth transition into our facility
• Develop and maintain relationships with referral sources, including physicians, hospitals, and other healthcare providers to ensure a smooth admissions process.
• Review and analyze admissions data to identify trends and opportunities for process improvement
• Collaborate with the Director of Nursing to ensure seamless communication and coordination between the admissions and nursing departments
• Ensure compliance with all regulatory requirements and maintain accurate records of patient admissions and discharges
• Utilize strong problem-solving skills to resolve issues and concerns related to admissions and patient care
