Job summary

Richmond Post Acute in Richmond, VA is seeking an experienced Admissions Director to lead the admissions process from inquiry to discharge, collaborating with nursing staff, patients, and families to ensure a smooth transition and high-quality care.

You will develop relationships with physicians, hospitals, and referral sources, analyze admissions data for improvement, and coordinate with the Director of Nursing to align admissions with clinical operations, while ensuring regulatory compliance.

Qualifications

• Bachelor's degree in a related field.
• Minimum 2 years of experience in an Admissions role in a Skilled Nursing or LTC setting.

Responsibilities

• Manage the admissions process, including coordinating with nursing staff, patients, and families to ensure a smooth transition into our facility
• Develop and maintain relationships with referral sources, including physicians, hospitals, and other healthcare providers to ensure a smooth admissions process.
• Review and analyze admissions data to identify trends and opportunities for process improvement
• Collaborate with the Director of Nursing to ensure seamless communication and coordination between the admissions and nursing departments
• Ensure compliance with all regulatory requirements and maintain accurate records of patient admissions and discharges
• Utilize strong problem-solving skills to resolve issues and concerns related to admissions and patient care
