Job summary

EASTERN HEALTHCARE GROUP is seeking an experienced Admissions Director in Stanardsville, VA to lead the admissions process for a skilled nursing facility. You will manage referrals, assessments, and documentation while coordinating with hospitals, physicians, and care teams to ensure smooth resident transitions.

You will verify insurance coverage and authorizations, ensure regulatory compliance, and maintain accurate records.

Qualifications

• High school diploma or GED; Associate or Bachelor's degree preferred.
• At least 2 years in healthcare admissions, discharge planning, case management, or related roles.
• Strong knowledge of Medicare, Medicaid, and managed care processes.
• Excellent communication, organization, and time-management skills.
• Proficient with Microsoft Office and electronic health record systems.

Responsibilities

• Manage full admissions and preadmissions process including referrals and documentation.
• Serve as liaison with hospitals, physicians, and referral sources.
• Coordinate with Central Intake and teams to ensure smooth transitions.
• Explain the admissions process to residents and families and provide tours.
• Verify insurance coverage and authorizations (Medicare, Medicaid, private).
• Ensure compliance with regulations and maintain accurate records in CRM/EHR.
• Support marketing, outreach, and community engagement efforts.
• Identify workflow improvements to enhance care and admissions outcomes.
