Job summary

Eden Senior Care, LLC's Edenbrook Rochester West is seeking an Admissions Director to lead patient intake at a skilled nursing facility. The role involves hospital rounds, evaluating high-acuity inquiries, and coordinating with case managers to ensure smooth admission and discharge planning.

Candidates should have experience developing relationships with hospital staff, social workers, and discharge planners, plus sales and referral generation skills.

Qualifications

• Knowledge of area hospitals and local healthcare organizations.
• Experience with physician and hospital staff relationship development.
• Sales and marketing experience for generating referral sources.
• Established relationships with local social workers and discharge planners.
• Excellent presentation and relationship-building skills.

Responsibilities

• Complete daily rounds in designated hospitals.
• Obtain new high-acuity inquiries and assess needs prior to admission.
• Evaluate inquiries, decide, and communicate to the case manager and admissions team for smooth transitions.
• Develop relationships with hospital social workers, physicians, discharge planners and others to understand needs.
