Qualifications

• Previous Admissions Director experience in a skilled nursing facility.
• Strong knowledge of admissions and referral processes.

Responsibilities

• Manage and coordinate the facility's admissions process from referral through admission.
• Review referrals and work closely with clinical and administrative teams.
• Communicate with hospitals, referral sources, residents, and families throughout the admissions process.
• Build and maintain strong relationships with referral partners.
• Coordinate admissions to help ensure a smooth transition into the facility.
• Maintain accurate admission documentation and follow-up.
• Work collaboratively with facility leadership to support census and occupancy goals.
