Autumn Lake Careers in New Jersey is seeking an experienced Admissions Director to lead the admissions process in our skilled nursing facility. You will coordinate referrals, collaborate with clinical and administrative teams, and guide families through transitions with clear, compassionate communication.

The ideal candidate has strong organizational skills, excels in a fast-paced environment, and builds lasting relationships with hospitals and referral partners to support census and occupancy

Qualifications

• Previous Admissions Director experience in a skilled nursing facility is required.
• Strong knowledge of the skilled nursing admissions and referral process.
• Excellent communication, organization, and relationship-building skills.

• Manage and coordinate the facility's admissions process from referral through admission
• Review referrals and work closely with clinical and administrative teams

• Maintain accurate admission documentation and follow-up
