Aventura at the Bay is seeking a dedicated, organized, and results-driven Admissions Director to join our team!

The Admissions Director is responsible for overseeing the admissions process and ensuring a smooth transition for new residents. This role works closely with hospitals, referral sources, families, clinical leadership, and facility staff to coordinate admissions while maintaining a high standard of customer service and compliance.

Responsibilities:

• Manage and coordinate the admissions process from referral through resident arrival

• Review referrals and clinical information to determine appropriateness for admission

• Coordinate admissions with hospitals, physicians, referral sources, residents, and families

• Build and maintain strong relationships with hospitals, healthcare providers, and community referral partners

• Conduct facility tours and communicate services, programs, and available accommodations to prospective residents and families

• Ensure all required admission documentation is complete and accurate

• Work closely with the clinical team to ensure appropriate placement and a smooth transition into the facility

• Maintain accurate admissions records and census information

• Monitor referral activity and follow up with potential admissions in a timely manner

• Support census growth and facility occupancy goals

• Ensure admissions are handled in accordance with facility policies, regulations, and applicable healthcare requirements

• Provide excellent customer service to residents, families, and referral partners

Qualifications:

• Prior experience in admissions, healthcare marketing, case management, or a related healthcare setting

• Prior experience in a skilled nursing facility (SNF) or long-term care (LTC) setting

• Strong knowledge of the skilled nursing/long-term care admissions process

• Excellent communication, organizational, and interpersonal skills

• Ability to build and maintain strong professional relationships

• Strong attention to detail and ability to manage multiple priorities

• Proficiency with computers and electronic medical record systems

• Ability to work independently and as part of a team
