Job summary

Chatham Hills Subacute Care Center is seeking an experienced Admissions Director to lead the admissions process and drive census growth while ensuring smooth transitions for residents and families.

You will develop referral relationships, oversee screenings and admissions, and collaborate with clinical teams to ensure appropriate admissions and compliance. This role offers a competitive salary with bonus potential in a fast-paced skilled nursing environment.

Qualifications

• Previous admissions, marketing, business development, or healthcare sales experience preferred
• Skilled nursing facility or rehabilitation center experience strongly preferred
• Excellent communication, customer service, and relationship-building skills
• Strong organizational and multitasking abilities
• Ability to work in a fast-paced environment
• Knowledge of insurance verification and healthcare admissions processes preferred
• Professional, motivated, and team-oriented

Responsibilities

• Manage and oversee all admissions activities for the facility
• Coordinate referrals, screenings, and admissions processes
• Develop and maintain strong relationships with hospitals, case managers, discharge planners, physicians, and community referral sources
• Conduct facility tours for prospective residents and families
• Collaborate with clinical and interdisciplinary teams to ensure appropriate admissions
• Track admissions data, referral activity, and census goals
• Ensure timely communication and follow-up with referral sources
• Assist with marketing and business development initiatives
• Maintain compliance with all federal, state, and facility policies and regulations
• Participate in community outreach and networking events
