You will manage referrals from intake to admission, verify insurance, and coordinate with clinical teams to ensure smooth transitions.

Qualifications

• Experience in admissions, intake, or healthcare coordination.
• SNF or healthcare experience strongly preferred.
• Strong communication and organizational skills.
• Ability to multitask in a fast-paced environment.
• Comfortable with insurance verification and documentation.

Responsibilities

• Manage incoming referrals and respond in a timely manner.
• Coordinate admissions from referral to admission, ensuring a smooth transition for residents and families.
• Communicate with hospitals, case managers, and internal teams to gather necessary documentation.
• Verify insurance and assist with eligibility when needed.
• Work closely with the Admissions Director and clinical team to review and accept appropriate referrals.
• Track and follow up on all leads to maximize census growth.
• Provide a strong first impression to families and referral sources.
