Admissions Coordinator SUMMARY: Under the general direction of the Program Director, you will develop an appropriate discharge plan, make recommendations for alternative levels of care, identifycost-effective care options and facilitateappropriate clinical follow-up. This work is done through patient assessment and engagement, chart review and collaboration with the healthcare team. You will monitor patient progress throughout the hospitalization, modifying discharge plans as needed and continually assess medical necessity. The Coordinator will also provide information to post-hospital vendors as needed to ensure ongoing recovery for the patient.

Admissions Coordinator JOB REQUIREMENTS:
