COUNTRY OAKS CARE CENTER is seeking a dynamic Admissions Coordinator to join our clinical team in California. You will support residents and families by planning activities, coordinating assessments, and participating in discharge planning.

Responsibilities include interviewing residents/families, documenting progress, and arranging transportation when needed. Requires 1 year in a Skilled Nursing Facility and 1 year in an Admissions role.

Responsabilidades

• Plan, develop, implement and evaluate activity programs for residents.
• Observe resident attendance, mood, behavior and involvement and document progress.
• Participate in community planning related to facility services and resident needs.
• Assist in discharge planning and development of activity care plans and assessments.
• Interview residents or family members to gather activity information.
• Involve residents/families in planning activities and objectives.
• Arrange transportation to other facilities when necessary.

We are looking for a full time Admissions Coordinator.

• Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facility.
• Observe resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated, by noting, reporting, and charting resident behavior.
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Participate in discharge planning, development and implementation of activity care plans and resident assessments.
• Interview residents or family members to obtain activity information.
• Involve the resident/family in planning activity programs when possible.
• Involve the resident/family in planning objectives and goals for the resident.
• Assist in arranging transportation to other facilities when necessary.
