Job summary

Grace Heights Health and Rehab is seeking an Admissions Coordinator to manage the admissions process for transitional rehab and long-term care, ensuring high customer satisfaction. You will coordinate communications with hospital discharge planners and families, conduct tours, and oversee paperwork and records.

Ideal candidates have 3–5 years in a healthcare admissions role and strong customer service, communication, and organizational skills. This role is based on-site with the facility.

Qualifications

• Associate's Degree preferred, but not required.
• 3–5 years in admissions in healthcare setting preferred.
• Excellent customer service, personality and drive.

Responsibilities

• Reach out to hospital discharge planners, families and referral sources.
• Liaise with residents and families on facility services and issues.
• Interview residents or guardians to determine eligibility for admission.
• Conduct tours of the facility and maintain a model room; refresh marketing materials.
• Assign accommodations for new admissions based on care needs and space.
• Ensure admissions paperwork is complete, accurate and timely; inform residents of responsibilities and benefits.
• Maintain admissions/discharge records and share occupancy data in meetings.
• Keep up to date with Medicare/Medicaid regulations to ensure compliance.

Job Summary:

Manages the admissions process for prospective and incoming residents requiring transitional rehab services or long term care and ensuring the highest level of customer satisfaction.

Duties & Responsibilities

• Proactively reach out to and respond to inquiries from hospital discharge planners, families, and other referral sources
• Serves as the initial liaison with residents and families on facility services and issues
• Interview residents or residents’ representative to obtain necessary personal and financial data to determine eligibility for admission
• Conduct tours of the skilled nursing facility/rehab area. Maintain a model room and update marketing materials for facility social media and various advertisements.
• Assign accommodations for new admissions related to nursing care needs, resident and residents’ family’s preference, availability of space, and other information.
• Ensure all admissions paperwork is completed thoroughly, accurately and on a timely basis. Informs prospective residents of their responsibilities, rights, and all benefits.
• Maintain records of admissions and discharges and compile occupancy census data. Participate in meetings to alert appropriate staff members of projected admissions, room changes, discharges, and appointments/tours.
• Maintain knowledge of Medicare & Medicaid regulations to ensure services are in compliance with professional boarding standards, state and federal regulatory requirements.

Required Qualifications
