The role focuses on patient satisfaction through first-24-hour rounds, clear communication of patient needs, and seamless coordination with the Transition Advisor and health center team.

• Collaborate with Transition Advisor and health center staff to ensure seamless admissions across centers
• Travel between centers within assigned territory as needed
