Qualifications

Responsibilities

• Conducts comprehensive assessments of patients upon arrival, gathering medical history and current condition to determine level of care.
• Collaborates with physicians, care managers, and healthcare staff to ensure accurate and timely patient admissions and documentation.
• Verifies patient information (demographics, insurance, medical records) to comply with admission protocols.
• Communicates admission process, care expectations, and patient rights to patients and families.
• Supports care coordination by initiating orders, treatments, and diagnostic tests per admission protocol.
• Maintains accurate EMR documentation of admission activities per standards and regulations.
• Participates in quality improvement initiatives related to the admission process.
• Provides education on facility services, treatment plans, and post-admission care.
• Adheres to privacy, safety, and infection control requirements during admissions.
