Responsibilities

• Welcome and admit new patients, establishing a positive first impression.
• Conduct comprehensive admission assessments and nursing interviews.
• Review, clarify, and enter physician/nursing orders accurately.
• Initiate personalized care plans with the interdisciplinary team.
• Communicate patient status and admission updates to the nursing staff.
• Educate patients and families on the admission process and services.
• Ensure admission documentation is complete in the EMR.
• Coordinate with physicians, therapists, and care teams for treatment planning.
• Identify changes in patient condition and potential risks.
• Collaborate with discharge planning to support seamless transitions.

In this role, you'll be the first point of contact for newly admitted patients, ensuring a smooth transition into our community through comprehensive assessments, care coordination, and compassionate support.Wage: $30.49-$37.20/hr.
