Job summary

Ebenezer Integrated Care & Rehab (EICR) in downtown St. Paul, MN, seeks a Licensed Practical Nurse (LPN) to deliver skilled nursing care in Transitional Care and Long-Term Care units. You will implement orders, assist with medications, and monitor resident health in collaboration with RNs.

The role requires MN LPN licensure, BLS/CPR certification, and the ability to work independently. Schedule includes day shifts, a two-week cycle, memory care, and alternating weekends/holidays.

Responsibilities

• Monitors unlicensed staff performance and provides guidance as needed.
• Updates RN with current observations and completes resident health documentation.
• Communicates changes in residents' conditions to relevant team members.
• May support RN by completing resident assessments, service agreements and care plans.

Job Overview

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community-from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation to make it easier for people to access what they need to be well.

We are seeking a Licenesed Practical Nurse - LPN for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This LPN role assists in meeting the residents' physical, emotional and psycho-social needs while adhering to all facility, state and federal guidelines, Ebenezer Standards and Policies.

This schedule includes;

• 48 hours per two weeks
• Day shift
• Block Schedule:
  Week 1: Thursday, Friday
  Week 2: Monday, Tuesday, Saturday, Sunday
• Memory Care Unit
• Every other weekend and holiday rotation

Duties:

Provides skilled nursing care such as; implementing practitioners' orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices
Lifts and transfer residents as needed, Initiates emergency treatment as needed
Monitors work and performance of unlicensed staff
Updates RN with current observation and completes documentation of resident health status
Communicates changes in residents conditions to relevant individuals
May support RN by completing resident assessments, service agreements and care plans

Required:

MN LPN licensure
AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers
Able to work independently
Good communication and interpersonal skills

Preferred:

Previous healthcare experience
Comfortable using computers, hand held devices, phones and fax machine
