Job summary

Capitol View Transitional Care Center, part of HealthPartners, seeks an RN/LPN for a night-shift role in a high-quality transitional care unit. The position is 0.5 FTE (20 hours/week) with schedule 10:30pm–7:00am and every other weekend, located in Saint Paul, MN.

You will provide direct patient care, manage medications, and support discharge planning while maintaining safety and documentation standards. Minnesota Nursing licensure and CPR certification are required.

Qualifications

• Graduate of an accredited school of nursing.
• Minnesota RN or LPN license in good standing.
• Current CPR certification.
• Reliable transportation to HealthPartners' Nursing Orientation in Bloomington.

Responsibilities

• Provide direct care to patients by assisting with ADLs and assessing status to guide care.
• Prepare room prep and discharge planning for patients.
• Answer call lights and follow up to ensure needs are met.
• Respond to questions from patients and families or direct to the appropriate person.
• Other duties as assigned.

We provide quality care to patients from 18+ years of age who are in need of a short-term stay. Our average length of stay is 17 days.
