Valor Hospice is seeking an Admission RN in Fort Worth, TX to perform admissions and provide nursing care at patients' residences. The role requires coordinating with the Interdisciplinary Team, families, and referring agencies, with flexible scheduling and on-call duties.

Monday–Friday, 9am–6pm, some variation by admissions and supervisor assignments. The RN must have an RN license, 2+ years clinical RN experience, and 1 year hospice experience, along with reliable transportation and compliance

• Education: Diploma/Associate/Baccalaureate in Nursing.
• Licensure: Current RN license and valid driver's license.
• Experience: 2+ years as RN in clinical care; 1 year hospice experience.
• Skills: Nursing skills, regulatory knowledge, good interpersonal ability.

Responsabilidades

• Perform admissions and administer nursing care for patients at home.

Job Details:

Job Location: FORT WORTH, TX 76104, Position Type: Full Time, Salary Range: $85,000.00 - $90,000.00 Salary, Job Shift: Day, Primary functions are to perform admissions and administer nursing care for patients of all ages in their place ofresidence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume theresponsibility for coordination of care. Admission RN is a salaried position and the RN must have the ability to work a flexible schedule. Primary working hours will be Monday thru Friday 9am-6pm. Hours may vary depending on time of admission. RN will be required to hold office hours as assigned by supervisor when not assigned to admissions. RN is required to participate in on-call scheduling as assigned and other duties as assigned by supervisor.

Essential Functions:

• Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, andenvironmental needs.
• Develop and document the plan of care in correlation with Assessment
• Provides nursing care utilizing infection control measures (OSHA)
• Work with Interdisciplinary Group/Team to ensure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Timely submission of all required paperwork.
• Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
• Promotes the Hospice’s philosophy and administrative policies.
• Meets productivity standards.
• Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
• Provides effective communication to patients, their family members, team members, and other health care professionals.

Qualifications:

• Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current State license as a Registered Nurse; Current driver’s license.
• Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
• Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.
• Good interpersonal skills.
• Transportation: Reliable transportation and valid auto liability insurance.
