Job summary

Community Healthcare of Texas is seeking an RN Admission Nurse to meet with patients and families, explain hospice services, and coordinate admissions in the Fort Worth area. The role includes conducting admissions assessments, planning nursing visit frequency, and coordinating with DME/RX and bereavement or social services as needed.

The position requires Texas RN licensure, a valid driver's license, and at least 1 year of nursing experience, with hospice case management experience preferred.

Qualifications

• Minimum 1 year of nursing experience.
• Hospice case management experience preferred (2+ years).
• Proficient computer skills and MS Office experience.

Responsibilities

• Meet with patients and families to explain hospice services and coordinate admission.
• Conduct admissions consultations and educate patients and families about hospice.
• Complete thorough admission assessment; establish initial nursing visit frequency and needs.
• Attend and report to Interdisciplinary Team regarding the patient.
• Complete paperwork and charting per policy.
• May conduct nursing or tuck-in visits.
• May train other admission staff on admission processes.
• May conduct supervisory visits on LVN or CNA staff.

Community Healthcare of Texas is currently recruiting for an RN Admission Nurse.

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services and coordinate admission of patients.

ESSENTIAL FUNCTIONS

• Conducts consultations for admission of patient services, including education of hospice services to patients and their families.
• Completes thorough assessment of patient at the time of admission; establishes/arranges initial frequency of nursing visits, DME/RX needs, and assesses the need for bereavement, social work, chaplain, and volunteer services.
• Attends and reports to the Interdisciplinary Team regarding the patient.
• Completes paperwork and charting thoroughly and accurately according to Policy.
• May conduct nursing or 'tuck-in' visits.
• May train other admission staff on admission processes.
• May conduct supervisory visits on LVN or CNA staff.

QUALIFICATIONS

• Must be a Registered Nurse in the state of Texas
• Hold a valid driver’s license
• 1 year of nursing experience required
• Strongly prefer 2+ years of experience in hospice case management
• Must have proficient computer skills and Microsoft Office Suite experience
